RN Case Manager


Leah has been a nurse since 2010 and prior to becoming a nurse, she was passionate about her work as a CNA caring for patients suffering from Alzheimer’s/Dementia. She graduated Magna Cum Laude with her BSN from Presentation College and is also a member of the Honor Society of Nursing and the International Honor Society in Social Sciences.

In her career she has worked with people of all ages and in multiple different specialties, some of which include Hospice, Transitional Care, Assisted Living and Memory Care, Home Care for adults and pediatrics and also Spinal Surgery. She takes great pride in the detail oriented and individualized care that she provides for each patient and holds great value in approaching healthcare from a holistic point of view, treating each person as a whole in body, mind and spirit. She highly values the relationships that we as caregivers build with patients and families and feel fortunate to have the opportunity to positively influence the lives of others.

She grew up on a farm in Victoria and now lives in the Western Suburbs with her husband, son and dog. In her free time she loves spending time with her family, cooking, exercising, learning new things, being outside (hiking, being on the lake and snowboarding in the winter). She is always running around doing something and also enjoy doing DIY projects around her house.
